SURRENDER A PET
Giving up an animal can be one of the toughest decisions to make. Whatever your reason, Willow Tree Sanctuary is here to help, without judgement. Please complete the below surrender form and one of our friendly team will be in contact. Please allow a few days while we assess suitability and find a foster carer.
It is a requirement by law that you notify a change of ownership by completing form C3A, listing Willow Tree Sanctuary as the new owner. You can find this form by clicking here. Without this form, we will be unable to help you. There is no fee to change ownership details. This form is required under the Companion Animals Act 1998 and Regulation 2008. It is the responsibility of the ‘old’ owner – the person selling or giving away the animal – to notify the change. Failure to do so may result in a fine of up to $880 or up to $5500 for a dangerous or restricted dog.
Please upload proof of identity and proof of ownership. This could be registration papers or a vet invoice that shows the animal name and your name. If you do not have papers with both names on it, please provide a stat dec stating you are the owner of the animal being surrendered. You can access a stat dec template here.

Make sure you have entered all fields with an asterix (*) next to it. The form will highlight missing fields and you will not be able to submit without completing them.
